Overview

Would you like to join our friendly and resolute team of clinicians and non-clinicians at a large NHS general practice based in Slough, Berkshire?

We are seeking to appoint one motivated and enthusiastic individual with a patient centred and excellent customer care experience to join our Clinical Administration team within the practice as a Care Coordinator & Safeguarding Administrator. About the role

You will be an integrated member of the Clinical Administration Team of twenty-two staff within the Practice and be a part of the wider Practice team and support the delivery of the Practice’s ambitions to deliver care of the highest quality and safety. You will help us care for our unique Slough population by managing the daily flow of confidential information and clinical records. You will be based at Farnham Practice but would be expected to travel to our other locations within Slough, to provide an efficient service to ensure the smooth and efficient running of the Clinical Administration department within the practice. About the Practice

Farnham Road Medical Group (FRMG) started as a small surgery and has grown over the years into an outstanding large training GP practice that is approved by NHS England (Health Education England). With four clinical sites based in Slough, we now have a team of over 100 people looking after 38,730 patients. The practice is committed to innovation in providing high quality health care for our patients and a protected working and learning environment for our staff. Our team includes pharmacists, nurses, physician associates, paramedics, MSK practitioners, Mental Health Practitioners, social prescribers, and care coordinators as well as GPs and a large Patient Services team. This resolute team provides an evolving healthcare and medical service to meet the varying needs of our patients and, in turn, provides what we believe is a sustainable model of modern General Practice. We have expanded our expertise and have transformed how we provide medical services to Slough’s diverse population.

Key Responsibilities

Work with GPs and MDT team within practices of FRMG to assist in the ongoing care of special groups of patients i.e, Learning Disabilities, End-of-life and Anticipatory Care Planning and signpost patients to relevant organisations for support.
Assist the Reception team in navigating patients into different clinical services.
To assist in call handling at Telephony Hub as part of care navigation and to help reception cover.
Be a central point of contact by managing the safeguarding mailbox and secure emails, processing all income information from start to finish and handling sensitive data with total discretion.
Maintaining accurate, secure, and up-to-date safeguarding record keeping and safeguarding databases.
Summarise, code, and circulate safeguarding documents to the appropriate clinicians, ensuring all information is tracked and highlighted where necessary.
Complete administrative reports for external agencies (e.g., MASH, Section 17/47) and prepare documentation for Child Protection conferences and MDT meetings.
Monitor DNAs codes for children regarding missed clinical appointments and refer to appropriate clinicians for follow up (e.g., usual GPs or Safeguarding Leads) in line with the Practice Safeguarding policies.
Engage in regular safeguarding supervision and case reviews to ensure you feel guided and supported in the role.

Your Skills/knowledge

Previous work within a healthcare setting or proven experience in a busy administrative role
Previous experience of working within a GP practice or working within or alongside Social Services, Local Authorities, or Children’s and Adult Services.
Confidence in dealing with colleagues from different teams and levels of seniority.
Experience of working directly in a community development context, adult health and social care, learning support or public health.
Excellent interpersonal, customer care, and communication skills, with the ability to work independently and as part of a team and perform effectively under pressure with diligence skills.
Good standard of literacy and numeracy.
Be initiative-taking with excellent timekeeping.
Good IT knowledge and exceptional knowledge of Microsoft Package.
Can build a rapport with our patients from different ethnic backgrounds and show empathy when interacting with them.
Able to work from as asset-based approach, building on existing community and personal assets.

Professional Qualifications

Level 5 qualification (i.e. Diploma of higher education Diploma of further education Foundation degree HND or equivalent professional experience.

Essential for the Role

Enhanced DBS check required.

Pay: Up to £14.50 per hour
