Full job description

Job Overview
We are seeking a dedicated and experienced Complex Care Coordinator to oversee and manage personalised care plans for clients within a senior care environment. The successful candidate will play a vital role in coordinating care services, supervising staff, and ensuring the delivery of high-quality, compassionate care. This role offers an opportunity to lead a team, develop customised care strategies, and contribute positively to the wellbeing of our clients.

Duties

Develop, implement, and regularly review personalised care plans tailored to individual client needs.
Coordinate and oversee daily care activities, ensuring compliance with organisational standards and regulations.
Supervise and support care staff, providing leadership and guidance to maintain high standards of service delivery.
Liaise with families, healthcare professionals, and external agencies to facilitate seamless communication and comprehensive care.
Organise training sessions for staff on best practices in senior care and safeguarding procedures.
Monitor client progress and adjust care plans accordingly to optimise wellbeing and independence.
Maintain accurate documentation of all care activities, assessments, and communications using IT systems.
Drive between different care homes or client locations as required to coordinate services effectively.

Requirements

Proven experience in senior or specialised care settings, preferably within a care home environment.
Supervising experience with demonstrated leadership skills in managing care teams.
Strong communication skills with the ability to liaise effectively with clients, families, and multidisciplinary teams.
Competent IT skills for managing electronic care records and administrative tasks.
Valid driving licence with the ability to travel between different locations as needed.
Knowledge of developing and managing personalised care plans aligned with regulatory standards.
Excellent organisational skills with the ability to prioritise tasks efficiently.
Compassionate approach with a genuine commitment to enhancing the quality of life for older adults requiring complex care support.

Pay: £24,420.00-£42,879.78 per year
