The Confederation, Hillingdon CIC works with General Practice and other healthcare providers to deliver its vision for Hillingdon to deliver the best primary care outcomes for patients in the whole of London. We are a not-for-profit community interest company. The Confederation works to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We deliver excellent clinical services ourselves both at scale and complementary to General Practice. Job Summary

The Children & Young People's (CYP) Co-ordinator will support children, young people and their families to access the right mental health and wellbeing support at the right time.

Working across Primary Care Networks, the postholder will act as a central coordinator between general practice, specialist NHS services, local authority teams, education, voluntary sector organisations and community services.

The role combines care coordination, service navigation, partnership working, pathway development and quality improvement to improve access to early intervention and reduce unnecessary escalation into crisis services.

The postholder will manage referrals, coordinate care, develop strong relationships across organisations and contribute to service improvement through data collection, reporting and evaluation.

Main Duties

Coordinate referrals across multiple services
Receive, review and triage referrals into the CYP programme
Build collaborative relationships across organisations
Provide advocacy, tailored support and empower self-management within sessions
Coordinate onward referrals to the most appropriate services
Support children, young people and families to navigate local services
Develop individual support plans where appropriate
Maintain regular contact with families whilst awaiting support
Escalate safeguarding or clinical concerns appropriately
Maintain accurate clinical records using digital systems
Represent the CYP Programme across Hillingdon

Partnership Working

Develop strong working relationships with:
GP Practices
Primary Care Networks
CAMHS
CYP Mental Health SPA
MHST
School Nursing
Family Hubs
Family Help Teams
Local Authority Services
Third Sector organisations
Community providers
Act as the first point of contact for advice regarding available CYP services.

Service Development

Support continuous improvement of the programme by:
Identifying pathway gaps
Creation of new initiatives to address pathway gaps
Mapping local services
Maintaining relationships with partners

Data and Quality Improvement

Record programme activity accurately
Collect outcome measures
Present data at PCN and programme meetings
Contribute to service evaluations

Engagement

Attend PCN meetings
Promote the CYP programme via engagement initiatives
Build awareness of referral routes

Governance

Participate in supervision and case discussions
Attend MDT meetings
Complete safeguarding and mandatory training
Follow information governance procedures

Health and Safety / Risk Management

The post-holder must comply at all times with all local Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.
The post-holder will comply with the Data Protection Act (1984) and the Access to Health Records Act (1990).

Patient Confidentiality

The post holder should not divulge patient information unless sanctioned by the line manager and required for the role.

Communication & Working Relationships

The post-holder will establish and maintain effective communication pathways at all times with project team members.

Special Working Conditions

The post-holder is required to travel independently between sites (where applicable), and to attend clinics and meetings hosted by other agencies.

Person Specification

Essential Criteria

Evidence of a high standard of written and spoken English (GCSE or equivalent)
Degree or equivalent Level 6 qualification in a relevant subject (e.g. Psychology, Social Work, Mental Health, Public Health, Medical & Ethics, Health & Social Care, Childhood Studies, Nursing or a related discipline).
Excellent communication skills
Able to work proactively and use own initiative
Ability to build relationships
Excellent organisational skills
Strong IT skills
Able to prioritise competing demands
Able to work independently
Able to analyse information and make appropriate decisions
Ability to use Microsoft Office, Teams and Excel
Experience working with Children and Young People
Experience working in Mental Health
Experience working within health, social care, education or voluntary services
Experience coordinating services or managing referrals
Experience working with multiple agencies
Experience maintaining confidential records
Experience using electronic patient record systems
Understanding of safeguarding
Full Driving Licence
Immunisation status - Hep B / MMR

Desirable Criteria

Coaching or counselling qualification
