Full job description

The Health and Wellbeing Practitioner will work within our health care team to provide high-quality health and wellbeing advice, support, and interventions to residents and patients, ensuring a person-centred approach to care. They will utilise their clinical expertise to promote overall wellbeing, working collaboratively with colleagues to enhance patient outcomes.

This role will work Monday to Friday on rotational shifts including four sleeping night shifts per month. No requirement to work weekends or Bank holidays.

Key Responsibilities

Deliver wellbeing support to residents in line with best practice guidelines.
Conduct health assessments and provide personalised health and lifestyle advice.
Support the clinical, health care team and weekly on site Doctor with monitoring chronic conditions and health promotion activities.
Provide education on preventive healthcare, mental wellbeing, and lifestyle choices.
Work closely with patients and multidisciplinary Physical Rehabilitation and Mental Health team to develop or support individualised care plans.
Maintain accurate and confidential health records in accordance with regulatory requirements.
Participate in rota-based sleeping night shifts (four per month) to ensure continuous care provision.
Give emergency physical and mental health support to patients in line with best practice guidelines night and day.
Respond to urgent health concerns and escalate appropriately within the clinical team during the day.
Assist with staff training on wellbeing and preventative care strategies.
Ensure compliance with health and safety policies and infection control procedures.
Present relaxation sessions and health education workshops to patients and residents, this may include water based relaxation.
Participate in guided health walks within the local countryside

Person Specification

Essential Criteria:

Current or previous professional registration with the relevant regulatory body (e.g. NMC or HCPC).
Demonstrable experience of delivering health and wellbeing interventions within a clinical or healthcare setting.
Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, colleagues and stakeholders.
Ability to work autonomously and collaboratively as part of a multidisciplinary team.
Flexible and adaptable approach to working patterns, including the ability to undertake sleeping night shifts monthly.
Commitment to delivering high-quality, patient-centred care and promoting a holistic approach to health and wellbeing.
Ability to maintain accurate clinical records and adhere to professional, ethical and organisational standards.
Sound understanding of safeguarding, confidentiality and information governance principles.

Desirable Criteria:

Additional training or qualifications in public health, mental health, health coaching or lifestyle medicine.
Experience of designing and delivering health education, wellbeing programmes or health promotion initiatives.
Knowledge of behaviour change techniques, motivational interviewing or coaching approaches
Experience of service development, quality improvement or evaluating health and wellbeing programmes.
Teaching, facilitation and presentation skills, with experience of delivering training to individuals or groups.
Experience of supporting or mentoring junior members of staff.
