About the Role

We are seeking a motivated and compassionate Mental Health Support Worker to deliver high-quality, person-centred support within a YMCA supported accommodation service for adult males.

This role focuses on preventing homelessness, promoting independence, and supporting individuals to achieve positive outcomes. Residents may present with mental health needs, substance misuse, trauma, and complex life experiences.

You will work in partnership with individuals to co-produce support plans, build resilience, and support progression towards independent living.

Key Responsibilities

Person-Centred & Co-Produced Support

Develop and review individual support plans based on assessed needs and personal goals
Work in a co-productive way, ensuring residents are actively involved in decisions
Deliver structured 1:1 keywork sessions focused on outcomes

Promoting Independence & Housing Stability

Support residents to maintain accommodation and understand tenancy responsibilities
Develop independent living skills (budgeting, cooking, self-care)
Assist individuals to move on to more independent housing options

Mental Health & Wellbeing

Provide support around mental health, emotional regulation, and wellbeing
Promote access to appropriate services (e.g. GP, mental health teams, substance misuse services)
Apply trauma-informed and psychologically informed approaches (PIE)

Risk Management & Safeguarding

Complete and regularly review risk assessments and safety plans
Identify safeguarding concerns and follow local safeguarding procedures
Respond appropriately to incidents, crisis situations, or relapse

Multi-Agency Working

Work collaboratively with local authorities, health services, probation, and third sector partners
Attend and contribute to multi-agency meetings and case reviews

Monitoring Outcomes & Compliance

Maintain accurate, timely records in line with HSG monitoring requirements
Evidence progress against outcomes (e.g. increased independence, improved wellbeing)
Contribute to service performance and continuous improvement

About You

You will be:

Committed to delivering person-centred, strengths-based support
Skilled in engaging individuals with complex needs
Able to work in a trauma-informed and psychologically informed way
Organised, reflective, and confident in managing risk
Passionate about supporting people to achieve independence

Essential Requirements

Experience working in supported housing, homelessness, mental health, or substance misuse services
Understanding of the Housing Support Grant (HSG) principles
Knowledge of safeguarding and risk management
Strong communication and record-keeping skills
Ability to work flexibly, including evenings/weekends
Enhanced DBS check (or willingness to obtain)

Desirable

Qualification in Health & Social Care (Level 2/3 or above)
Experience working within hostel or supported accommodation settings
Knowledge of Welsh policy frameworks (e.g. Rapid Rehousing, PIE, trauma-informed practice)
