Care Coordinator (Full-Time) Domiciliary Care

We are seeking an organised, proactive, and compassionate Care Coordinator to join our domiciliary care team. The Care Coordinator will be responsible for planning and managing staff rotas, ensuring safe and efficient delivery of care, and maintaining excellent communication with clients, carers, and stakeholders. The successful candidate will be confident using a Digital Rostering system to build rotas, manage schedules, and support operational efficiency. This role is central to the smooth running of our care service. You will work closely with the Registered Manager and community care team to ensure every client receives quality care while staff are deployed effectively and according to their skills, availability, and preferences.

Working hours – Monday - Friday 08.30 -17.00

Flexibility to work in the community to cover visits as and when required

Main duties of the job

Key Responsibilities Rostering & Scheduling

Create, maintain, and update staff rotas using Digital Rostering System.
Ensure rotas are completed in a timely manner and meet client care planrequirements.
Manage last-minute changes, sickness, and emergency cover.
Ensure continuity of care and minimise missed calls or late visits.

Job responsibilities

Care Coordination

Act as the main point of contact for care staff regarding schedules.
Liaise with clients and families to manage changes, queries, or concerns.
Allocate appropriately skilled staff to service users based on needs, risk assessments, and preferences.
Support the induction and shadowing process for new carers.

Compliance & Quality

Ensure care plans, visit notes, and documentation are accurate and up to date.
Run reports and monitoring checks to ensure KPI's are met and payroll information is accurate.
Assist with monitoring visits, assessments, reviews, spot checks, and audits as needed.

Communication & Team Support

Work with Registered Manager to support operational performance of the service.
Work with external professionals (GPs, district nurses, social workers)
Communicate with commissioning managers (LCC, ICB)
Escalate safeguarding, risk, or quality concerns appropriately.
Ability to prepare care plan’s furthermore, dealing with updates and also start new packages when required.
Flexibility to provide support as a senior carer to cover visits were needed.
Conducting assessments and quality reviews on a regular basis.
Supervising and managing the care staff, ensuring they are adhering to the required quality and legal standards within their job role.
Carrying out spot checks on staff.
Working closely with the Registered Manager and Area Manager to effectively plan the care service.
Providing guidance, training and shadowing to the care team.
To represent the Company in a professional manner at all times, on the telephone, face to face or in written communication.
Professionally and promptly answer telephone calls, ensuring people are spoken to in a polite and respectful manner.
To maintain confidentiality at all times and carry out the Company’s Confidentiality Policy.

Person Specification Essential

Experience in care coordination within a domiciliary care setting.
Proficient in using Care RosteringSystem.
Understanding of CQC standards and domiciliary care operations.
Excellent communication and customer service skills.
Strong organisational and multitasking ability.
Proficient IT skills, including care management systems and MS Office.
Ability to work on-call and participate in a rota.
Strong attention to detail.
Excellent problem-solving skills.
The ability to build a professional working relationship with service users and colleagues.
Excellent understanding of English language, both written and spoken.
A valid driving licence and a car owner.
An even-tempered and patient nature, having the ability to adapt to each individual need using empathy where required.

Qualified to NVQ level 3 in Health and Social Care
Ability to work under pressure in a fast-paced environment.
