Full job description

To provide evidence based clinical assessment and interventions for people with a mild, moderate or severe learning disability who are known to the team.
To work with individuals in need of support to help them develop coping strategies that minimise distress.
To support staff teams and families to deliver a holistic and person centred approach
There is requirement to work in a variety of settings and in collaboration with a number of different agencies.
The role requires the post holder to be included in the on-call rota that covers weekends and bank holidays between the hours of 08:00hrs and 18:00hrs
To supervise junior colleagues and support students on placement.
To be prepared to disseminate assessment outcomes in the most appropriate forum. This may include meetings, staff workshops and training

Provide assessment of individuals, including risk assessments, using agreed clinical and therapeutic formats to deliver planned interventions as required.

Contribute to the development of person-centred support plans in partnership with service users, carers and the multi-disciplinary team.

To act in an advisory capacity to individuals, families and support staff, providing evidence based information and training.

To be proactive in offering interventions that are the least restrictive option.

To determine the appropriateness of referrals and clinical effectiveness.

Ensure effective communication and develop close working relationships between service users, families, carers and all relevant agencies, both internal and external.

To participate in a supportive multi-disciplinary team.

To be able to work autonomously.

Ensure adequate written records are kept in line with agreed policies of the Rotherham, Doncaster and South Humber NHS Foundation Trust. To accurately record all visits and contacts, including regular input to the clinical database.
