Job Description – Ageing Well Coordinator

Responsible to: Senior Ageing Well Coordinator
Place of Work: Hybrid working; working across, Harborough Field Medical Centre, Parklands Surgery, Higham Ferrers Surgery, Spinney Brook Medical Centre, The Cottons Medical Centre, Marshalls Road Surgery, The Meadows Surgery, Nene Valley Surgery, Home visits, Community settings and Northamptonshire Carers Head office is at 123 Midland Road, Wellingborough. Countywide travel expected.

Note: This role requires an enhanced DBS check and full adherence to Health & Safety, GDPR, and safeguarding protocols.

Job Summary

You will represent Northamptonshire Carers as part of a multidisciplinary Ageing Well team which includes, Age UK, Alzheimer’s Society, Adult Social Care and Northamptonshire Healthcare Foundation Trust (NHFT) based in Primary Care Networks across Northamptonshire. As part of this team, you will offer holistic assessment and support to achieve the best outcomes for people aged 65+ with complex long term health conditions at risk of hospitalisation or becoming dependant on statutory or institutional care.

Supporting older people to stay well and live well.

Key Responsibilities

1. Direct Support & Multidisciplinary Casework

Act as a key point of contact in the Ageing Well team, caring out holistic assessments and case management for older adults at risk of hospitalisation.

· To take referrals from assigned GP surgeries, arrange, coordinate and undertake an initial home visit to carry out a holistic assessment of need for individuals referred to Ageing Well. Agree with them the course of action that is to be taken making relevant referrals to local service provision.

· Conduct home visits and attend frailty clinics; carry out basic health observations (e.g., BP, oxygen levels). Recognising signs of deterioration and reporting these to relevant health professional. Ensuring all details of these observations are recorded on clinical records.

· Where identified as helpful and agreed with the patient, to visit them in their homes, taking a laptop to allow the patient and family to participate in a multi-disciplinary meeting (MDT)/ extended patient review with health professionals on a Microsoft Teams Virtual Call.

· To carry out low level equipment assessments, and order equipment if appropriate.

· You will be expected where required to carry out support for unpaid Carers alongside your allocated patient work. This maybe for patient who are Carers, Carers of patients allocated to you, or Carers identified by Ageing Well colleagues. Support may include registering Carers, identifying initial needs, Carers assessments and where required referring into both Northamptonshire Carers services external services and liaising with other professionals.

· To manage a diverse, complex and fluctuating caseload of carers and patient. It is the worker’s responsibility to maintain this caseload at a manageable level (with Co-ordinator’s support).

· To work to action plans with identified timescales and targets. Whilst supporting Carers it is important to take an empowering and preventative approach to promote independence and improve the sustainability of the caring role as appropriate.

· To maintain up to date accurate written and computer records using shared IT systems (e.g., clinical databases, Organisational databases, Microsoft Teams), in accordance with local policies and data protection and confidentiality legislation. Document and monitor all aspects of Ageing Well input and service delivery, supporting data collection and auditing.

· Work in a person-centred, empowering way to reduce crisis interventions and promote independence.

· To be aware of all community and voluntary sector services (not just within Northamptonshire Carers) provided in surrounding areas, to assess for these services and make onward referrals

· To work as part of a team, participating in team meetings, team building, supervisions, training and activities. Appropriate to Northamptonshire Carers and the Ageing Well role

· To be responsible for ensuring all patient identifiable information is stored and disposed of appropriately, and that Information Governance and Confidentiality procedures are adhered to

· To ensure that reasonable care is taken at all times for the health, safety and welfare of yourself and other persons, and to comply with the policies and procedures relating to health and safety and safeguarding within the organisation.

To undertake other duties as may be required by the CEO/Trustees of Northamptonshire Carers

2. Partnership & Community Engagement

Build effective working relationships with GP surgeries, primary care networks (PCNs), and partner agencies (e.g., Adult Social Care, NHFT).

To promote the work of Northamptonshire Carers and Ageing Well, building local networks with potential referrers and GP Surgeries.

The tasks and responsibilities shown above are not exhaustive and should be regarded only as a guide. The jobholder will be expected to undertake any reasonable activities according to the needs of the service at the time. These will be subject to periodic review and may be amended to meet the changing needs of the service and role. The jobholder will be expected to participate in this process, and Northamptonshire Carers would aim to reach agreement as regards to any changes.
