Overview

We are seeking a dedicated and proactive Care Co-ordinator to join our dynamic team, committed to delivering exceptional senior care and support. In this vital role, you will oversee the coordination of care plans, supervise care staff, and ensure the highest standards of service within a caring environment. Your leadership and communication skills will help create a positive atmosphere where residents feel valued, safe, and well-supported. Purpose of Position

All aspects of Care Management in the designated areas, accountable to the Registered Manager.

Hours

Monday – Friday 9-5 plus on-call duties

Principal Responsibilities

1. To assist the Registered Manager in all aspects of care management.

2. Contribute to the development and growth whilst ensuring SureCare’s commitment to quality at all times

3. Contribute to the research, sales and development of new areas of services provision

4. Actively promote and sell the services of SureCare

5. To assist in the recruitment of Care Workers observing current SureCare procedures and policies.

6. To maintain accurate records in line with SureCare’s policies and procedures relating to the handling, storage and if necessary, deletion, of personal details to ensure compliance with the General Data Protection regulations for Care Workers and Service Users. keep all records updated using computer and manual systems and implementing any security measures to protect the personal data.

7. Produce accurate care worker rotas on a weekly basis, ensuring all calls are covered

8. Observe legislative requirements according to the Working Time Regulations.

9. Observe Service User and Care Worker confidentiality.

10. Accurately and appropriately match Care Workers to Service Users.

11. Become competent in all areas of office management with exceptions only at the Registered Managers discretion.

12. Produce data/reports as required.

13. Assist with problems that may arise between Care Workers and Service Users effectively on a daily basis.

14. Observe SureCare’s Policy in relation to issues surrounding confidentiality.

15. Communicate and liaise with all office personnel, social workers, Service Users and Care Workers etc.

16. Assist with “on call” duties at weekends, weekday nights and bank holidays.

17. Deputise in the Registered Manager’s absence.

18. Undertake any other duties in line with the basic objectives of the post.

Capable of accepting responsibility and coping with emergencies
Positive attitude to own health and work attendance record
Ability to implement and review care plans and work with complex cases
Basic computer literacy
Maintain staffing levels at all times, ensure that the telephones are manned from (Insert correct times e.g. 9am -5.00pm)

Desirable attributes:

NVQ2/3 in Care qualification
Car driver with access to a car

All staff are required to respect the confidentiality of all matters that they might learn in the course of their employment. All staff are expected to respect the Data Protection requirements under the General Data Protection Regulation (GDPR).

All staff must ensure that they are aware of their responsibilities under the Health and Safety at Work, etc Act 1974.
