Service Coordinator – Doncaster Supported Living

Base Location: Doncaster Office Pay Rate: £13.52 per hour Hours: Full‑time, 40 hours per week (Monday to Friday)

If so, we want you to join our team as a Service Coordinator in our Doncaster Supported Living Services at IBC Healthcare.

As a Service Coordinator, you will play a vital role in ensuring high‑quality support, meaningful engagement, and strong leadership across our Doncaster Supported Living services.

What You’ll Be Doing

Person‑centred planning — Designing and implementing tailored support plans that promote independence, inclusion, and wellbeing.
Activity leadership — Facilitating meaningful, engaging activities based on individual preferences and abilities.
Team leadership — Working alongside Support Workers, modelling best practice, and maintaining high standards of care.
Supervising and mentoring Shift Leaders to ensure consistent, high‑quality support.
Deputising for the Deputy Manager during periods of leave.
Conducting staff supervisions, team meetings, and performance reviews.
Providing on‑call support on a rotational basis.
Developing, reviewing, and updating bespoke care plans.
Creating and promoting Positive Behaviour Support (PBS) plans.
Completing Nourish compliance checks and weekly audits.
Maintaining strong communication with healthcare professionals.
Supporting medication management, including administration, ordering, auditing, and staff training.
Conducting daily and weekly spot checks across supported living homes.
Leading collaborative work with multidisciplinary teams (MDTs).
Participating in staff recruitment, interviews, and competency assessments.
Assisting with regulatory inspections and ensuring compliance.
Overseeing operational tasks such as food orders, weight tracking, healthcare matrix updates, and fire safety audits.

What We’re Looking For

A compassionate, proactive individual committed to improving lives.
Experience overseeing a care team supporting adults with learning disabilities (Deputy Manager level desirable).
Proven experience delivering engaging activities for adults with learning disabilities and additional needs.
Minimum 3 years’ experience in a Support Worker role.
Strong leadership, communication, and organisational skills.
A full UK driving licence.
