Full job description

Care Coordinator and Care Assessor

Job overview

To assess service users’ care and support needs, develop and review care plans, coordinate care delivery ,matching care workers appropriately to service users based on their individual needs, preferences, risks, skills, experience, and compatibility to promote safe, effective, and person-centred care delivery. To ensure services are in accordance with organisational policies and regulatory requirements.

Key Responsibilities:

Care Coordination

Coordinate and allocate care visits according to service user needs and staff availability.
Ensure continuity of care wherever possible.
Monitor schedules and respond to changes, emergencies, or staff absence.
Liaise with care workers to ensure care plans are implemented correctly.

Service Delivery and Quality Assurance

Conduct spot checks, observations, and service reviews.
Monitor care records and ensure accurate documentation.
Investigate concerns, incidents, complaints, and safeguarding matters as required.
Support compliance with regulatory and contractual standards.

Communication and Partnership Working

Maintain effective communication with service users, relatives, health professionals, local authorities, commissioners, and multidisciplinary teams.
Attend review meetings and contribute to care planning discussions.
Provide updates regarding care changes and outcomes.

Staff Support and Supervision

Support induction and on-going development of care staff.
Deliver or coordinate training where appropriate.
Provide guidance to care workers on best practice and care delivery expectations.

Compliance and Record Keeping

Maintain accurate electronic and paper records.
Ensure compliance with confidentiality, data protection, safeguarding, medication, and health and safety requirements.
Support audits and regulatory inspections.

Person Specification

Essential

Experience working within health and social care.
Knowledge of care assessments and care planning.
Understanding of safeguarding and person-centred care.
Good organisational and communication skills.
Competence in record keeping and care management systems.
Ability to prioritise workload and work under pressure.
Full driving licence and access to a vehicle (if community-based).

Desirable

Level 3–5 qualification in Health and Social Care (or working towards).
At least two years previous experience in domiciliary care coordination or assessment.
Knowledge of local authority and CQC requirements.

Care Assessment and Planning

Carry out initial assessments and reassessments of prospective and existing service users.
Identify individual care, support, moving and handling, communication, and risk management needs.
Develop person-centred care plans and risk assessments.
Ensure assessments reflect outcomes, preferences, dignity, independence, and choice.
Review care packages regularly and update documentation where required.

Service user satisfaction.
Staff retention and rota efficiency.
Compliance and audit outcomes.
