Are you an organised, proactive and patient-focused administrator looking for your next challenge?

We have an exciting opportunity to join our Children's Physiotherapy Team as a Referral to Discharge Coordinator, playing a vital role in supporting the delivery of high-quality paediatric physiotherapy services across MPFT.

This is a key position within the team, responsible for coordinating the patient journey from referral through to discharge, ensuring children and families receive a responsive, efficient and compassionate service. Working closely with the Paediatric Physiotherapy Team Leads, Physiotherapists, Physiotherapy Assistants and the wider multidisciplinary team, you will help ensure that patients are seen at the right time, in the right place, by the right clinician.

You will:
Manage and coordinate the paediatric physiotherapy referral process from receipt to discharge.

Monitor waiting lists and clinic capacity to support achievement of service targets.

Schedule appointments and ensure urgent referrals are prioritised appropriately.

Act as a key communication point between families, clinicians, secondary care providers and external agencies.

Maintain clinical records within the RiO patient management system.

Support service improvement projects and contribute ideas to enhance patient pathways.

Provide day-to-day supervision, guidance and training to administration colleagues.

Handle patient enquiries and resolve issues promptly and professionally.

Support clinicians by ensuring clinics run efficiently and available capacity is fully utilised.

Main Duties of the job:
You will play a key role in coordinating the paediatric physiotherapy service, managing referrals, appointments and patient pathways from referral through to discharge. Detailed job description and main responsibilities

We're looking for a friendly, organised, and proactive individual to help keep our service running smoothly and provide a positive experience for children, young people, families, and professionals.

In this role, you will manage patient referrals and records, book and coordinate appointments, respond to appointment enquiries, and support clinical administration processes, including onward referrals. You will help ensure patients are seen within agreed waiting times and service targets, working closely with colleagues to resolve any issues and maintain an efficient service.

A key part of the role is building positive relationships with a wide range of people, including children and young people, parents and carers, colleagues, healthcare professionals, and partner organisations. You will communicate confidently and compassionately, using a non-judgemental approach and handling sensitive information appropriately.

You will use a variety of digital systems and software, including Microsoft Office and electronic patient record systems such as Rio, ensuring information is accurate, up to date and managed in line with service requirements. You may also be required to produce reports and support the ongoing development of digital processes.
