Overview

We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality nursing care to patients in their own homes. The ideal candidate will possess strong leadership qualities, excellent communication skills, and the ability to develop personalised care plans. This role offers an opportunity to make a meaningful difference in patients’ lives by delivering professional, empathetic, and organised care within a community setting. The position is paid and suitable for individuals committed to promoting health and wellbeing through personalised nursing services.

Responsibilities

Deliver comprehensive nursing care tailored to individual patient needs within their home environment
Develop, implement, and regularly review personalised care plans in collaboration with patients and their families
Monitor patient health status, administer medication, and perform routine health assessments
Provide education and support to patients and carers regarding health management and treatment procedures
Coordinate with multidisciplinary teams to ensure continuity of care and optimise patient outcomes
Maintain accurate documentation of all care activities, observations, and interventions in accordance with regulatory standards
Lead by example in maintaining high standards of hygiene, safety, and infection control practices
Offer leadership and guidance to junior staff or carers involved in patient care where necessary

Requirements

Female candidate.

Nursing qualification or relevant caregiving experience.

Good communication skills in English.

Kind, patient, and trustworthy.

Willing to work as a live-in caregiver.
