Position Overview
The Care Manager will play a key role in the day-to-day management and coordination of Rayman Healthcare's domiciliary care service, working under the direction and oversight of the Registered Manager.
The postholder will support the Registered Manager in ensuring that care is delivered safely, effectively and in accordance with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, CQC requirements, safeguarding procedures, contractual requirements and Rayman Healthcare's policies and procedures.
The Care Manager will provide operational leadership to the care and coordination teams, with particular responsibility for care delivery, staff performance, care planning, safeguarding, quality assurance, rostering and ensuring that concerns are identified and escalated appropriately.
This is a non-registered management position and does not replace the statutory responsibilities or regulatory accountability of the CQC Registered Manager.
Key Responsibilities
- Support the Registered Manager with the effective day-to-day running of the domiciliary care service.
- Provide visible and effective leadership to care coordinators, senior care staff and care workers.
- Ensure staff understand their roles, responsibilities and expected standards of conduct and performance.
- Promote a positive culture of accountability, professionalism, teamwork and person-centred care.
- Identify operational concerns promptly and escalate significant risks or concerns to the Registered Manager.
- Deputise for the Registered Manager in operational matters when required, within the limits of the role and delegated authority.
- Support the continued development and growth of the service while ensuring quality and safety are maintained.
- Support the Registered Manager in maintaining compliance with CQC regulations, fundamental standards and relevant health and social care legislation.
- Undertake regular quality assurance audits, including care plans, risk assessments, medication records, daily notes, call monitoring, staff files and other areas of service delivery.
- Identify areas requiring improvement and develop appropriate action plans in conjunction with the Registered Manager.
- Monitor completion of agreed actions and escalate areas of continued non-compliance.
- Support preparations for CQC inspections, local authority monitoring visits and other external audits.
- Promote continuous improvement and ensure learning from incidents, complaints and feedback is embedded into practice.
- Complete and oversee initial assessments, care plans, risk assessments and regular reviews.
- Ensure care plans accurately reflect people's needs, preferences, choices and desired outcomes.
- Ensure care is delivered in accordance with agreed care plans and commissioned hours.
- Monitor changes in people's needs and ensure these are appropriately documented, communicated and escalated.
- Ensure care workers have the appropriate skills, knowledge and competency to meet the needs of the people they support.
- Support the management and coordination of complex or changing care packages.
- Conduct spot checks and observational assessments to monitor the quality of care being delivered.
- Promote a strong safeguarding culture throughout the service.
- Ensure safeguarding concerns are recognised, documented and immediately escalated in accordance with company procedures.
- Take appropriate immediate action to protect people where there are concerns regarding abuse, neglect or avoidable harm.
- Support the Registered Manager with safeguarding enquiries and internal investigations.
- Liaise with local authorities, social workers and other professionals where delegated by the Registered Manager.
- Ensure staff understand their safeguarding responsibilities and know how to report concerns.
- Support the recruitment, induction and ongoing development of care staff.
- Conduct staff supervisions, competency assessments, probation reviews and appraisals as delegated.
- Address day-to-day performance concerns and escalate significant or repeated concerns to the Registered Manager.
- Support capability, disciplinary and investigation processes where required.
- Ensure staff complete mandatory and specialist training appropriate to the people they support.
- Maintain oversight of care coordination and rostering to ensure safe and effective deployment of staff.
- Ensure appropriate staffing levels are maintained to safely deliver commissioned care.
- Monitor missed calls, late calls, continuity of care, travel time and other factors affecting service delivery.
- Ensure care workers are appropriately matched to people based on their needs, preferences, skills and competency.
- Work closely with the coordination team to manage staff availability, sickness, annual leave and emergency cover.
- Respond effectively to changes in care packages and operational pressures.
- Support the safe management of medication within the service.
- Ensure staff providing medication support are appropriately trained and assessed as competent.
- Respond appropriately to day-to-day concerns raised by people receiving care, relatives, staff and professionals.
- Support the Registered Manager with the investigation of complaints, incidents, accidents, medication errors and allegations.
- Develop positive professional relationships with people receiving care, their families, representatives and advocates.
- Liaise with social workers, commissioners, district nurses, GPs, community teams and other healthcare professionals.
- Support staff to use electronic care management systems effectively.
Qualifications and ExperienceEssential
- Previous supervisory, coordination or management experience within domiciliary care or adult social care.
- Good working knowledge of CQC regulations and the fundamental standards.
- Good understanding of safeguarding adults, risk management and person-centred care.
- Experience of supervising and supporting care staff.
- Experience completing or reviewing care plans and risk assessments.
- Experience with quality assurance and auditing within a care environment.
- Excellent communication and interpersonal skills.
- Strong organisational skills and ability to manage competing priorities.
- Confidence in addressing poor practice and staff performance concerns.
- Competence in using electronic care management systems and Microsoft Office applications.
- Full UK driving licence and ability to travel as required by the service.
Desirable
- Level 3 or above qualification in Adult Care, with a willingness to work towards a Level 5 Diploma in Leadership and Management for Adult Care.
- Previous experience as a Care Manager, Deputy Manager, Senior Care Coordinator within domiciliary care.
- Experience working with local authorities, NHS professionals and other commissioners.
- Experience supporting complex care packages.
- Experience participating in CQC inspections, local authority monitoring or quality assurance processes.
