Full job description

The Service Coordinator is an operational and patient-facing role within the E4 Primary Care Network, working across all five PCN practices. The post-holder has two core areas of responsibility: supporting the coordination and day-to-day running of PCN services, including Enhanced Access and Same Day Access sessions, under the direction of the Service Delivery Manager; and acting as a dedicated coordinator of patient care, helping patients navigate their care journey, coordinating input from the wider primary care team, and supporting proactive, personalised care for those with long-term conditions or complex needs.

This is not a senior or line-management role. The post-holder will escalate operational, staffing, clinical, or safeguarding concerns to the Service Delivery Manager as appropriate.

Key Responsibilities

Patient Care Coordination

● Act as a single point of contact for a caseload of patients across the PCN's practices, helping them navigate and access the right care, at the right time, in the right place.

● Coordinate personalised care and support planning for patients with long-term conditions, multiple co-morbidities, or complex needs, working closely with PCN practices to achieve the targets associated with the care provided.

● Bring together the different elements of a patient's care and support plan, ensuring information is shared appropriately between practices, clinicians, and other services.

● Support patients to understand and manage their own health and care, signposting to social prescribing link workers, health and wellbeing coaches, and other community and voluntary services as appropriate.

● Coordinate and track referrals, follow-up appointments, and investigations, helping ensure patients do not fall through gaps in their care.

● Support the delivery of PCN clinical services.

● Support multidisciplinary team meetings and case discussions, helping prepare information and following up on agreed actions.

● Maintain accurate, up-to-date patient records on EMIS and SystmOne, ensuring care coordination activity is clearly documented.

● Help identify patients who could benefit from proactive care coordination, working with GPs and practice teams to build and manage a caseload.

Service Coordination Support

● Support the day-to-day coordination of PCN services, helping ensure the smooth running of clinics and service sessions.

● Assist with the creation and maintenance of clinics within the EMIS system, ensuring accurate set-up in line with service requirements.

● Support rota coordination for PCN services, including helping identify and cover short-notice gaps where required, and escalating unfilled shifts to the Service Delivery Manager.

● Help maintain adequate stocks of stationery, clinical, and other supplies, flagging shortages or cost-effectiveness concerns.

● Carry out daily safety checks relevant to the service (for example, fridge temperatures, emergency drugs, and clinical equipment checks) as required, accurately recording these on Practice Index.

● Produce accurate reports relating to the service delivery in a timely manner ensuring that relevant deadlines are met.

● Support audit activity, including data collection, updating audit spreadsheets, and tracking actions to completion.

● Help coordinate information relating to complaints and significant events, ensuring accurate documentation, and escalating concerns promptly.

● Act as a key holder for service premises where required, following organisational security procedures.

Compliance and Governance

● Follow PCN policies, protocols, and clinical governance requirements at all times.

● Escalate any non-compliance, safeguarding concerns, or risks immediately to the Service Delivery Manager.

● Maintain awareness of relevant PCN contractual frameworks (for example, the Network Contract DES) as they relate to service and patient care coordination.

Pay: £14.50 per hour
