Full job description

We are seeking a proactive, organised and analytical individual to join our Primary Care Network as a Population Health & Neighbourhood Coordinator.

Hindley PCN constitutes 6 member GP practices with a collective registered population of 49,021 patients.

Hindley PCN is a proactive and inclusive Primary Care Network, serving a diverse population across Hindley, Platt Bridge and Ince communities.

At Hindley PCN we recognise that health outcomes are not the same for everyone. Many people in our communities’ face barriers such as socio-economic disadvantage, cultural differences, or limited access to healthcare services. Our approach prioritises reaching out to these underserved groups through tailored, community-based interventions. By working closely with local organisations, faith groups, and voluntary sector partners, we aim to build trust, improve access, and deliver care that meets the unique needs of our diverse population. Addressing health inequalities is central to everything we do, ensuring that all individuals have the opportunity to achieve their best possible health

Our team includes a Clinical Director, PCN Manager, Operational Manager, Mental Health Team, Advanced Clinical Practitioners, Enhanced Nurse, Experienced Nurse, First Contact Physiotherapists, General Practice Assistant, Clinical Pharmacists, Care Coordinators and Social Prescribers, working collaboratively to deliver person-centred care and support population health improvement.

Key Responsibilities

· Undertake clinical system searches to identify high-risk and vulnerable patient groups.

· Support NHS data quality improvement initiatives and maximise data uptake across member practices.

· Create dashboards, reports and KPI monitoring information.

· Produce contractual performance reports and PCN dashboards.

· Support outreach and community clinics including patient check-in.

· Manage Ledgers, recalls and patient follow-up processes.

· Provide administrative support to the PCN team.

· Coordinate meetings, agendas, minutes and action logs.

· Maintain shared task boxes and failsafe processes.

· Ensure compliance with information governance and GDPR.

Essential Criteria

· Excellent IT and Microsoft Office skills.

· Experience using GP clinical systems.

· Experience producing reports and dashboards.

· Strong organisational and communication skills.

· Experience coordinating meetings and action logs.

Desirable Criteria

· NHS or Primary Care experience.

· Power BI experience.

· Knowledge of population health management.

· Understanding of PCN contractual requirements.
