Care Coordinator – Job Description (Domiciliary Care)Job Title

Care Coordinator

Reports To

Registered Manager / Care Manager

Job Purpose

The Care Coordinator is responsible for organising and coordinating the delivery of safe, effective, and person-centred domiciliary care services. The role ensures that care packages are appropriately planned, staffed, and monitored to meet the needs, preferences, and outcomes of people receiving care in their own homes. The post holder will work closely with care workers, service users, families, healthcare professionals, and external agencies to maintain high standards of care and support compliance with CQC requirements and relevant legislation.

Key Responsibilities

Coordinate daily care visits and ensure care packages are delivered safely and consistently.
Create and maintain care rotas, ensuring appropriate staffing levels and continuity of care.
Match care workers with service users based on individual needs, preferences, skills, and compatibility.
Support the development, review, and updating of person-centred care plans and risk assessments.
Monitor the quality of care provided through communication with service users, families, and care workers.
Respond promptly to changes in service users’ needs and update care arrangements accordingly.
Act as a key point of contact for service users, relatives, care workers, and professionals.
Manage and resolve queries, concerns, and complaints in a professional and timely manner.
Ensure all care records, daily notes, and documentation are accurate, complete, and maintained in line with GDPR requirements.
Report safeguarding concerns, incidents, and accidents in accordance with company policies and local safeguarding procedures.
Support the recruitment, induction, training, and supervision of care workers.
Monitor care worker attendance, punctuality, and compliance with company procedures.
Conduct spot checks, service reviews, and quality monitoring visits as required.
Assist with audits, inspections, and continuous improvement activities.
Ensure services are delivered in line with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, CQC Fundamental Standards, and other relevant guidance.
Promote independence, dignity, choice, and respect for all individuals receiving care.

Person SpecificationEssential

Experience working within a domiciliary care or social care environment.
Understanding of person-centred care and safeguarding responsibilities.
Knowledge of CQC requirements and relevant health and social care legislation.
Strong organisational and time management skills.
Excellent communication and interpersonal skills.
Ability to prioritise workloads and manage changing demands.
Good IT skills and experience using electronic care management systems.
Ability to work effectively with care workers, service users, families, and professionals.

Desirable

Level 3 Diploma/NVQ in Health and Social Care or equivalent.
Experience in coordinating domiciliary care services.
Experience supervising or supporting care workers.
Knowledge of care planning, risk assessments, and quality assurance processes.
Experience supporting people with learning disabilities, autism, dementia, or complex care needs.

Working Conditions

The Care Coordinator must promote safe working practices, ensure risks are identified and managed appropriately, report incidents and concerns, and support compliance with health and safety procedures.
