Job Overview

The Medicines Co-ordinator will take a central role in reviewing, improving, and implementing the practice repeat prescribing policies to ensure that all staff are aware of the correct procedures to run a safe, cost effective, efficient, and patient friendly, repeat prescription system.

We are looking for an enthusiastic administrator who has excellent organisational, multidisciplinary and communication skills with experience working in the general practice setting.
Provide coordination and navigation to patients across the organisation requiring medication support through processing medication requests, shared care monitoring and carrying out Medicines Management projects.
Support patients with their understanding of the pharmacy services provided by the Medicines Management Team.

Main duties of the job

Aims:

Determine accuracy of both hospital and GP prescriptions with regard to medication
Provide independent information to patients with regard to medicines management and through assessment initiate further support/input from other healthcare professionals where appropriate
Implement procedural and service changes relating to prescribing to GP practice staff to ensure best practice is adopted in accordance with local guidelines and legislation
To link into local clinical governance structures to report on risk management issues relating to the prescribing process
To support the surgery in the planning and delivery of medicines management services
Daily Nursing home Prescriptions monitoring prescription requests
Appliance/wound care contractor request monitoring
Prescription queries from healthcare professionals / More complex queries from healthcare professionals
Daily queries/information requests from clinicians
Daily queries from all outside agencies including patients
Action any new guidelines from LMMG/CCG
Red/Amber drug request queries

Additional responsibilities will include ensuring that

ALL clinicians have access to and utilise the OptimiseRx decision support software.
The practice utilises the Eclipse Live patient support platform.
The Medicines Co-ordinator will be the main contact point for non-clinical medication related queries from pharmacies, care homes, and other health care professionals, to improve the accuracy of patient medication records.
The Medicines Co-ordinator will be a main link between the practice and the Primary Care Networks Clinical Pharmacist(s) and / Pharmacy Technician(s) if applicable.

Clayton Brook Surgery is along-established GP practice based in Clayton Brook and works closely with a partner practice Moss Side Medical Centre in Leyland, working together to provide high-quality, patient-centred healthcare to our local community.

Clayton Brook Surgery and Moss Side Medical Centre have a great cohesive team ethic and working environments. We have GP Partners, Salaried GPs, 1 Advanced Nurse Practitioner, Practice Nurse, Nurse Associate, HCA, Medicines Co-ordinator, a Business Manager, Practice Manager and an Operations Manager and a team of Receptionist / Administrators. We are also Training Practices with our Partners being GP Trainers / Educators.
