Job Overview
We are seeking a compassionate and dedicated Caregiver to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have a strong background in case management and care planning, particularly for those with specific needs such as dementia or hospice care. As a Caregiver, you will play a vital role in enhancing the quality of life for our clients by delivering personalised care and support.

Responsibilities

Assist clients with daily living activities, including personal hygiene, meal preparation, and medication administration.
Develop and implement individualised care plans tailored to each client's needs.
Provide companionship and emotional support to clients, fostering a positive environment.
Monitor clients' health and behaviour, reporting any changes to the appropriate healthcare professionals.
Maintain accurate records of care provided and any significant observations.
Ensure a safe and clean living environment for clients by performing light housekeeping duties.
Collaborate with nursing home staff and other healthcare providers to ensure comprehensive care.

Skills

Strong case management skills with the ability to develop effective care plans.
Experience in behaviour management techniques to support clients with challenging behaviours.
Proficient in first aid and emergency response procedures.
Knowledge of dementia care practices and the ability to provide specialised support.
Experience compassion during end-of-life care.
Excellent communication skills, both verbal and written, with the ability to build rapport with clients and their families.
