1. Description

Post Title: Support Worker

Due to the specific requirements of this role, this post is exempt under the Equality Act (2010), Part 1, Schedule 9 (Genuine Occupational Requirement).

1.2 Main Purpose of Post

To ensure that the service provision of the charity are conducted in a manner which furthers the aims of Me, Myself & I Neath Port Talbot, delivering on a person centred care approach for people living with dementia to access a day break provision.

1. SERVICE SPECIFIC

Service provision including;

Provide and lead on an approach which encompasses person centred care for all guests.
Support the organisations service manager with duties such as:

Provide support to guests attending Day Break provision and support team members with Friendship Afternoon, as required.

Including, but not limed:

Greeting guests on arrival and ensuring safe storage of belongings
Mealtime support if required
Provide personal care if or when required
Refreshments throughout the day
Provide support offering meaningful activities
Support guest to leave ensuring they have all belongings
Completion of paperwork, ensuring accurate and adequate information is entered
Keep a safe and comfortable environment for guests, volunteers and staff. Ensure spaces are kept clean, sanitised and free from any health & safety hazards
Health & Safety –
Report any Hazards, Incidents or Accidents to the correct member of staff
Raise any Safeguarding concerns to your Team Leader as soon as possible
Promote a person centred care approach at all times ensuring you support volunteers and Academy students through leading by example.
Support Care Partners to access information, support and guidance. Refer to Service Manager and escalate concerns.
Help promote an environment of co-production and collaboration where people’s voices are heard and their views and or their complaints are taken seriously and addressed and people feel they matter.
In addition to the above duties and responsibilities, the post holder must be prepared to undertake such additional duties which may result from changing circumstances, but which may not necessarily change the general character, level of responsibility or grading of the post.
To be responsible for own continuous professional development and ongoing learning.

3. GENERIC PROFESSIONAL RESPONSIBILITIES

To assist in ensuring that effective two-way communication methods are in place and that all guests, volunteers and paid staff receive relevant information in a timely manner.
To assist in contributing to improved performance through the take up of training and development opportunities.

This is an outline description of the temporary job as it presently exists. This temporary job description will be examined periodically and updated to ensure that it relates to the job as then being performed or to incorporate whatever changes are proposed by MMI. Any such changes will be subject to consultation with the post holder. It will be the Charity`s aim to reach agreement to reasonable changes, if agreement is not possible, the charity reserves the right to insist on changes, following consultation

WORK EXPERIENCE

ESSENTIAL

Experience of working in social care
Experience of working within dementia care
