We're looking for an experienced Care Coordinator to join our friendly office team here at Willows Home Care.

Willows was started in 2016 by Charlotte Wright after 23 years working as a paramedic. She wanted to create a home care company that genuinely put people first, both the clients we support and the team who care for them. Fast forward to today, and we're proud to support more than 40 clients across our local community.

Charlotte now works alongside her daughter, Beth, our Assistant Manager, and together they've built a business that still feels like family. We're not a big corporate company where you're just another number. Everyone knows each other, everyone mucks in, and we genuinely look out for one another.

If you've worked in care coordination before, you'll know that no two days are ever the same. It can be busy, challenging and sometimes unpredictable. The difference at Willows is that you're never expected to deal with it all on your own.

We've been doing this role ourselves for the last 10 years, so we know exactly what it involves. And that hasn't changed. Charlotte and Beth still do the job, so you'll be working alongside them every day, not reporting to managers who have forgotten what it's like on the front line.

When things get busy, they roll their sleeves up and help. Whether that's covering calls, supporting the care team or helping to solve problems, they're right there with you. You'll never be left to figure things out on your own because the people supporting you genuinely understand the role.

The salary we advertise is the salary you'll be paid, and the shift pattern listed is the one you'll work—no hidden expectations or surprise extra hours. Overtime is always available if you want it, but it's never expected. We know a good work-life balance matters, so when you're off, you're off. Your days off are yours to enjoy.

Working Hours & Pay

Salary: £29,536 per year.
You'll work 3 midweek days (9:00am-5:00pm) in our Penwortham office. Your working days will be agreed with you.
On each of your office days, you'll also be responsible for the on-call service outside of office hours.
You'll take part in an alternate weekend on-call rota, from Friday evening until Monday morning. This can be managed from home, as the office is closed over the weekend.

The Role

No two days are the same. You'll be coordinating care, supporting the office team and getting out into the community to support clients and staff when needed. It's a varied role where you'll make a real difference every single day.

Unlike many larger providers, we still get out and deliver care ourselves. Charlotte and Beth regularly cover care visits when needed because we believe good leaders should never lose touch with the people they support. From time to time, you may also be asked to help with care visits, so a full UK driving licence and access to your own vehicle are essential.

What you'll be doing

Coordinating day-to-day care and responding to new enquiries.
Creating and reviewing care plans.
Carrying out staff supervisions.
Speaking with clients and families to gather feedback.
Completing audits and risk assessments.
Supporting with general office administration.
Helping us continue to improve and maintain the high standards we're proud of.

About You

We're looking for someone who:

Has previous Care Coordinator experience.
Is organised and able to manage a busy workload.
Is friendly, approachable and professional.
Can work independently but enjoys being part of a team.
Is confident using Microsoft Office and a care management software.
Wants to be part of a company where people genuinely support one another.

Essential Requirements

Previous experience as a Care Coordinator.
Level 3 Diploma in Health & Social Care (minimum).
Full UK driving licence and access to your own vehicle.
Good computer skills.
Right to work in the UK.
