Full job description

Overview

We are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will be responsible for organising and supervising care plans for our clients, primarily within the community care setting (own home).

This role requires excellent communication skills, leadership qualities, and the ability to coordinate effectively with care staff, clients, and their families. A valid driving licence is essential, as the position involves travelling between various locations.

The Care Coordinator will play a vital role in ensuring high standards, ensuring high level of care delivery, supporting the front line team when required and supporting on-call across all service provisions.

Duties

Develop, implement, and review personalised care plans tailored to individual client needs within care home and senior care environments
Supervise and support care staff to ensure delivery of high-quality care in accordance with organisational policies and legal requirements
Coordinate schedules for carers, ensuring timely attendance and optimal coverage for all clients
Act as the primary point of contact for clients, families, and care teams, maintaining clear and compassionate communication at all times
Conduct regular assessments to monitor client wellbeing and adjust care plans as necessary
Organise training sessions for staff to promote best practices in eldercare and senior support services
Maintain accurate documentation using IT systems, including care records, schedules, and incident reports
Ensure compliance with health and safety regulations within the workplace
Cover on-call shifts (evening / weekends) Rota basis
Travel between different locations as required to oversee service delivery and attend meetings

Experience

Proven supervisory or leadership experience within a care home or senior care setting is highly desirable (Minimum Level 2 - option to work towards Level 3)
Experience in developing or managing care plans for elderly or vulnerable adults
Strong driving record with a valid driving licence is essential due to travel requirements
Excellent communication skills, both written and verbal, with the ability to liaise effectively with diverse stakeholders
Proficiency in IT systems utilised within healthcare or social care environments
Prior experience supervising staff or managing teams in a healthcare context is advantageous
Knowledge of relevant legislation relating to eldercare and safeguarding procedures
