Shore Medical is a GP Super Partnership with 6 practices across Poole and Bournemouth. We aspire to offer exceptional care to our 58,000 patients and are innovative in our approach to developing new teams and pathways to ever improve how general practice is delivered. We have a friendly and supportive team of more than 200 staff, with over 40 GP’s, Pharmacists, Paramedics, Nurses, Mental Health specialists and our range of administrative staff. We have a great social side to our team with Summer and Christmas parties, running and paddle boarding groups as well as many other events throughout the year.

The Opportunity

Shore Medical Group are seeking Care Navigators to join our friendly team at our Wessex Road Surgery, Ashley Cross.

This is a phone-based role focusing on providing a safe and competent triaging service by assessing the patient’s condition and ensuring they are directed/signposted to most appropriate healthcare professional or service, including advice on how to manage their symptoms at home.

Job type- Permanent Full time x 1 /Part time x 2

Hours of work- 8.00am-6.30pm (must be able to work Monday, Tuesday, Wednesday and Friday afternoons

Hourly rate - £12.90 per hour

Key Responsibilities

Identify potential serious problems and bringing them to the immediate attention of the designated clinician
Develop and maintain an in-depth knowledge and undertaking of the service provided within Shore Medical and in the wider health community, be able to use this knowledge to guide the patient to the service that is most likely to meet their needs, whether inside or outside the practices
Build strong relationships with all direct team members, clinicians and recognise healthcare professionals within the Shore Medical Group and external healthcare professionals as a resource to be used appropriately
Support the ‘putting patients first’ agenda around care planning and case management to support patients in their management of care and avoid unnecessary hospital admissions
Promptly and accurately booking SDA appointments with patients via telephone
Triaging Klink’s efficiently and safely under the guidance of clinicians and available pathways
Engaging with patients via outbound calls and messaging system in an empathetic manner.

About You

Proven experience in a similar phone-based role
A confident communicator on the phone, able to communicate empathetically while dealing efficiently with sensitive issues
High attention to detail and accuracy applied in all administrative work
Knowledge of the Data Protection Act and handling confidential information is desirable
Experience in an NHS/Primary Care background is desirable
Proficiency in Microsoft Office and IT literacy to learn new software (SystmOne)
