Full job description

Job Purpose:

To provide high-quality, person-centred support to young people aged 16–17 living in supported accommodation. The role involves helping young people develop independent living skills, access education or employment opportunities and navigate emotional, practical and social challenges in preparation for independent adulthood.

Key responsibilities:

1. Young person-centred support

· Build positive, trusting relationships with young people based on mutual respect and clear boundaries.

· Deliver tailored support plans, focusing on independence, emotional well-being and practical life skills.

· Promote engagement in education, training, employment, and community activities.

· Encourage healthy routines, including personal care, cooking, cleaning and budgeting.

2. Safeguarding and risk management

· Ensure a safe and supportive environment for all young people.

· Identify and report safeguarding concerns promptly in line with policy.

· Contribute to risk assessments and safety planning.

· Support young people in understanding and adhering to house rules and placement agreements.

3. Key working and case management

· Act as a key worker for assigned young people, maintaining regular sessions and tracking progress.

· Contribute to the development and review of support plans and placement goals.

· Liaise with external professionals including social workers, education providers, and health services.

4. Daily accommodation duties

· Support the running of the household, including supervising shared areas and encouraging cleanliness.

· Monitor and log young people’s well-being through daily reports and incident records.

· Support the move-in and move-out process, including inventories, welcome packs, and transition planning.

5. Teamwork and professional practice

· Work collaboratively with colleagues to ensure consistent support.

· Attend team meetings, handovers, and training as required.

· Maintain professional boundaries and confidentiality at all times.

· Uphold the organisation’s values, policies, and procedures.

Person specification:

Essential:

· Experience working with young people, preferably those who are care-experienced or at risk of homelessness.

· Understanding of safeguarding and child protection procedures.

· Ability to build relationships and support young people with varying needs.

· Strong communication and record-keeping skills.

· Flexibility to work shifts, including evenings and weekends.

Desirable:

· NVQ Level 3 in Health and Social Care (Children and Young People) or equivalent.

· Experience in supported accommodation or semi-independent living services.

· Knowledge of relevant legislation (e.g. Children Act 1989, Leaving Care Framework).
