Full job description

About The Reach Partnership

The REACH partnership is an umbrella organisation of Northamptonshire based youth counselling and emotional wellbeing support agencies. The REACH partnership delivers a range of counselling and support services to young people. The partnership has been commissioned to deliver mental health support to young people transitioning from adolescent to adult services.

Thriving in Life

Service Six & Youth Works, on behalf of the REACH partnership of Youth Counselling services In Northamptonshire and working in partnership with Northamptonshire Healthcare Foundation Trust, are commissioned by North Northamptonshire Council to deliver a service to support young people aged 11-18. This support will be available both in their schools and local communities and will include assemblies, groupwork and 1:1 support.

The service is based on the Thrive framework:

1. GETTING ADVICE In partnership with others, promote and encourage the use of services and support that is available, utilising community assets.

2. GETTING HELP Provide a range of accessible and evidence interventions, delivered early to prevent escalation. Onward referral if required.

3. GETTING MORE HELP Close working with specialist services to promote and integrated approach to the care of young people.

4. GETTING RISK SUPPORT Safeguarding and support for complex and vulnerable young people.

The ethos and culture of the service is solution-focused, strengths-based, personalised, flexible and will be able to work with a complex set of needs, regardless of diagnosis.

There are four teams of support workers, one in each of Corby, East Northants, Kettering & Wellingborough.

Job Purpose

To provide support, guidance and information to young people aged 11 to 18 within a specified geographical area.

General Duties

1. Within the team develop, through co production with young people, population and community level health promotion activity addressing:

Emotional Health & wellbeing
Developing and maintaining healthy relationships
Making healthier choices
Reducing risk taking behaviour
Transition to adulthood & being work ready

2. Develop relationships in allocated schools, offering a weekly drop in service

3. Develop relationships in allocated communities

4. Deliver a range of quality population and community level health promotion activity and support services including:

School or community assemblies
Group workshops
1:1 support

5. Promote universal access to digital advice, help and information

6. Provide casework support to a caseload of clients, offering ongoing support with related issues through direct support and signposting.

7. Support the development of a local database of agencies regarding signposting.

8. Ensure all monitoring outcomes data is recorded and input

9. Contribute to the presentation of monthly statistical reports and any written reports, case studies and summaries of service delivery

10. Participate in regular team meetings and ensure that team meeting minutes are read and acted upon

11. Contribute to sharing practice and specialised information and support through county briefings and collaborations.

12. Collaborate effectively with GPs, local NHS providers (e.g. CAMHS etc. tbc) in order to receive and make appropriate referrals

Organisation Duties

13. Adhere to and remain current with Service Six policies and procedures which include but are not limited to: Equality & Diversity; Safeguarding; Health & Safety; and Confidentiality.

14. Attend appropriate clinical supervision.

15. Undertake all relevant administrative tasks.

16. Attend any other additional meetings and training as required.
