Full job description

About the Role

As Care Co-ordinator, you'll be responsible for the day-to-day coordination of care delivery across your locations — ensuring rotas are fully staffed, records are accurate and audit-ready, and care plans reflect each service user's current needs. You'll work closely with the Team Leader and Registered Manager to maintain compliance standards and support a culture of continuous improvement across the team.

Key Responsibilities

Organise and manage staff rotas to ensure all commissioned care hours are covered, escalating any gaps to the Team Leader immediately.
Audit care folders, applications, and client records against company policy and CQC's fundamental standards on a [weekly/monthly] basis, logging and resolving any gaps within agreed timescales.
Ensure individual care plans are created, kept up to date, and communicated clearly to each client and their family.
Support the supervision of support workers alongside the Team Leader, including spot checks to confirm care delivery meets required standards.
Handle complaints and grievances in the first instance, escalating appropriately and maintaining a clear written record of actions taken.
Promote independence and person-centred choice for every client.
Maintain accurate, audit-ready records across rostering, care planning, and communication logs, using [Care Planning Software] and Microsoft Office.
Support the Team Leader and Registered Manager in implementing quality improvement actions arising from audits or CQC inspections.

Essential Criteria

NVQ/QCF Level 3 in Health & Social Care.
Minimum 2 years' experience in Supported Living, including direct experience coordinating rotas or care delivery (not solely assisting a coordinator).
Demonstrable experience conducting audits, compliance checks, or quality assurance activity — this is a core requirement for this role.
Working knowledge of CQC's fundamental standards and current assessment framework.
Practical understanding of safeguarding adults procedures and the Mental Capacity Act 2005.
Enhanced DBS check (on the Update Service, or willingness to apply).
Full UK driving licence and access to a vehicle.
Right to work in the UK.
Confident using care planning software and Microsoft Office applications.

Desirable Criteria

Experience handling formal complaints or grievance processes.
Experience supporting or leading a quality improvement action plan.
