Full job description

Care Co-ordinator — Supported Living & Homecare

Location: Morecambe

Hours: Full-time, 40 hours per week, usually worked over five days in seven. Will involve evening/weekend work.

Pay: £14.60 per hour. On-call duties are paid separately in line with current company arrangements.

A care service can only be reliable when the coordination behind it is reliable too. We are looking for an organised, calm and person-centred Care Coordinator who can help our supported living and homecare services run safely every day.

Primarily, you will co-ordinate staffing and schedules, communicate with professionals and service users, keep information accurate, help managers follow through on actions, support care planning and quality checks, respond to changing situations and be a practical point of contact for staff, people we support and professionals.

It is not a desk-only role. Care is a 24-hour service, so there will be occasions when you need to attend a service, support an urgent situation, take part in on-call or provide hands-on care where other contingency arrangements have been exhausted. Applicants therefore need to be comfortable with the practical realities of frontline care.

What you will do

Coordinate day-to-day operational information across supported living and homecare, keeping managers updated on staffing, risks, incidents, appointments, changes in need and actions requiring follow-up.
Support rota and scheduling activity, including sickness, leave, continuity, skill mix and urgent cover, using standby, redeployment and escalation arrangements appropriately.
Help complete and maintain assessments, support plans, risk assessments, reviews and other care records, ensuring changes are recorded promptly and escalated for management oversight where required.
Be a practical first point of contact for staff, people we support, families and professionals, communicating clearly and escalating decisions outside your authority.
Participate in the organisation-wide on-call rota, triaging calls, coordinating responses, escalating serious concerns and attending in person when reasonably required.
Provide occasional direct support when genuinely necessary for safety or continuity, including personal care, moving and handling, wheelchair support, medicines, meals and community support, subject to training, risk assessment and competence.

What we are looking for

Good adult social care experience and a clear understanding of person-centred, safe and respectful support.
Strong organisation and confidence using digital care records, rota/scheduling systems, email, shared documents and spreadsheets.
A calm, solutions-focused approach when plans change, staff are unavailable or urgent decisions need coordinating.
Confidence communicating with adults, families, care staff, managers and professionals.
A willingness to work across both supported living and homecare and learn the different operational requirements of each service.
Reliable travel between services. A full UK driving licence and access to a suitable vehicle are normally expected unless an effective reasonable alternative is agreed.
A Level 3 qualification in Adult Care / Health and Social Care is desirable. Applicants without it should be willing to work towards an appropriate qualification if required.
