Full job description

Job Summary

We are seeking a motivated and compassionate Healthcare Assistant (HCA) to join our friendly and dynamic practice team. The successful candidate will support the delivery of high-quality clinical care and health promotion services to our patient population.

The post-holder will work closely with GPs, Practice Nurses and the wider practice team to assist in delivering a range of clinical duties within primary care. The role requires excellent communication skills, strong organisational abilities and a commitment to providing high standards of patient care.

The successful candidate will be supported through regular supervision, training opportunities and continued professional development.

Main Duties and Responsibilities

The Healthcare Assistant will work within their competencies and training to support clinical service delivery across the practice.

Clinical Responsibilities

The post-holder will undertake duties including:

Phlebotomy
Blood pressure monitoring
NHS Health Checks
Height, weight and BMI recording
Urinalysis
ECG recording
Smoking cessation support
Diabetic foot checks
Spirometry (if trained)
Post-operative wound care and dressings
Immunisations and vaccinations (where appropriately trained)
Pill checks and medication support
Stock monitoring and clinical room preparation

Training will be provided in accordance with practice requirements and service needs.

Patient Care

Support patients with chronic disease monitoring and health promotion.
Recognise signs of deterioration or abnormal findings and escalate appropriately.
Provide lifestyle advice and health education to patients.
Maintain patient dignity, privacy and confidentiality at all times.
Refer patients to appropriate clinicians or external services when necessary.

Quality & Outcomes Framework (QOF)

Support the practice in achieving QOF and enhanced service targets.
Ensure accurate coding and recording of clinical information within the clinical system.
Assist with patient recalls, audits and health promotion initiatives.

Administrative & Record Keeping

Maintain accurate and contemporaneous patient records.
Ensure clinical templates and documentation are completed appropriately.
Assist with data collection for audits and quality improvement projects.
Participate in maintaining clinical equipment and stock control processes.

Collaborative Working

The post-holder will:

Work collaboratively with the multidisciplinary practice team.
Develop and maintain positive working relationships with colleagues and external healthcare professionals.
Communicate effectively with patients, carers and healthcare staff.
Recognise personal limitations and seek support or guidance when required.
Contribute positively to team meetings and service development.

Professional Development

The post-holder will:

Participate in annual appraisals and regular supervision.
Undertake mandatory training and continuing professional development.
Maintain competencies relevant to the role.
Keep up to date with current clinical guidance and practice policies.
Participate in educational opportunities relevant to primary care.

The Healthcare Assistant will:

Demonstrate professionalism and accountability in all areas of work.
Contribute to maintaining high clinical standards within the practice.

Maintain strict patient confidentiality in accordance with GDPR and NHS Information Governance requirements.
Ensure all patient information is handled securely and appropriately.
Adhere to practice confidentiality and safeguarding policies.
