Role: Support Worker - Complex Care
Hours/ shifts: full time hours per week and part time hours per week (Generally weekdays) £14.00 per hour with enhancements for weekend and waking nights £15.50 and £13.00 per hour for sleeping nights. Shifts are variable and allocated on a rotational basis including - day-time, nigh-time and weekend shifts.
Location: Based at Client’s home in the South West
Responsible To: Mark Rodd, Director Elm Care Management and the allocated Case Manager for the client.
Background:
- To work with all clients, providing care and support during the day, when at social activities and when holidays and short breaks.
Job Summary:
- To work as a support worker role, assisting and where appropriate enabling the client to lead a full
and active life as possible, always maximising their physical and psychological well-being and safety.
- To provide full care and assistance with personal care including assistance with hoisted transfers if required.
- To encourage, support and assistance with leisure activities and engagement with family members.
- To support the client with communication and managing risks when areas of concern are noted.
- Some assistance with personal care required.
- All specialist training is provided which might include bowel care, catheter, dialysis, UTI and specialist equipment and implementing therapy programmes.
Purpose of post
To work as part of a team providing support and care for our clients, adhering to all safety guidance, maintaining the clients physical and mental health wellbeing.
To facilitate our clients wellbeing by adopting the appropriate approach as advised by the Case Manager and/or treating therapists.
Personal attributes:
To have a caring nature, whilst also the ability to show initiative, and use skills to navigate day-to-day events and be able to problem solve.
To work as part of a team and communicate appropriately with all team members.
Respect the need for confidentiality when the client or their family speak/communicate on matters of a private and personal nature.
Requirements:
A successful application should have a nationally recognised Care Certificate or complete this within 4 months of commencing the post. It is desirable to have experience in care and or also NVQ Level 2 or equivalent in care, including experience of. working with clients with learning or physical disabilities.
Preferably hold a full driving licence and happy to drive on motorways as required.
Training and support maybe provided online, eg via Zoom, access to IT equipment will be essential.
The post holder must:
Have a desire to learn new strategies and knowledge for working with clients.
To be keen to follow instructions and therapy programmes and take guidance.
Be able to show reliability and trustworthy nature.
Have the ability to work using IT for email and support worker documentation.
To have access to IT away from the client’s home, ie an alternative secure site to ensure online training and access to client’s updates can be received.
Accompany the client to appointments and/or therapy sessions.
Key Support worker expectations:
Our clients require assistance for all activities of daily living, including personal care, bowel and bladder care, dialysis and therapeutic, social and leisure tasks.
To provide full assistance to the clients to ensure a high standard of hygiene is maintained.
Our clients should be treated with respect and dignity in all activities, at all times.
Communication should be at all times respectful, caring and aligned to with their cognitive abilities,
acknowledging their individuality.
Our clients should be encouraged to make decisions/choices over their own life, with acknowledgement of the client’s cognitive and physical impairments. Ensuring that you are aware of the client’s vulnerability and take precautions to protect it.
To enable the client to use community facilities including social groups, local facilities and attend community events as appropriate.
To ensure opportunity is given to the client to participate in activities as identified by others but balancing the need for periods of rest & changes in position to maintain the clients health and well-being.
To ensure that any training needs are discussed with the Case Manager and set agreed timeframes in which to complete mandatory online training.
To adhere to guidelines when taking the client out in a vehicle, including providing documented evidence of insurance for business use.
Treating therapists have been appointed and assessments have been carried out have provided programmes, strategies and support to aid the client with their well-being. Support worker training will be provided.
Report any hazards or health and safety concerns, breakages or equipment faults as soon as possible.
Client specific training will be required for bowel management, in addition therapy support and assistance will be provided to enable support staff to be confident with equipment use.
Read and keep updated with all Case Management policies and procedures.
Key Goals:
To help our clients in all activities of daily living, including routines and social activities, so as to maximise their quality of life.
To support recommendations made by the treating therapists.
Petty Cash/Expenses:
Be responsible for any “petty cash” which may be made available to you.
Checking and recording this on arrival and when leaving shift.
Keep a record of expenditure with receipts, ensuring these are sent to the Deputy Team Leader, Team Leader or Case Manager along with an expenses form on or before the last working day of the month.
Pay Monthly:
A form will be forwarded to you for completion regarding details required.
Hours will be collected in order to be sent to payroll on the last working day of the month. You will be paid by BACS on the 5th of the month.
Contract:
There will be a six-month probationary period. Regular reviews will occur throughout the probationary period alongside a structured induction period. It will be a requirement to have completed the online mandatory training during the probationary period. A formal review will occur to ensure all parties agree to progress from a probationary contact.
A letter of appointment will be given after receiving two references and current DBS number is provided or registration is completed and a formal contract when you have completed your 6 months probationary period to the satisfaction of the Team Leader and Case Manager.
Annual leave is calculated based on the standard 5.6 weeks pro rata per annum and awarded from 1st January till 31st December N.B The calculation of annual leave will be dependent on the number of contracted hours and is calculated by our HR software.
Overtime hours can be completed but will not incur any additional holiday entitlement. Overtime will be paid at the standard hourly rate for that shift/day/time.
I.D. Cards:
A company I.D. card will be provided and you will be expected to wear it whenever on shift.
You will be required to provide a photo with a plain background for this purpose, the Team Leader can do this for you.
Liaison:
It is mandatory that all support workers must attend staff meetings and meetings with the Case Manager and/or Team Leader, held no more frequently that once every six weeks, unless there is an urgent need to request an earlier meeting, you will be given an appointment for these meetings.
Therapist will complete face to face and online training sessions, attendance is mandatory.
You will receive payment for attending meetings or trainings sessions if these hours are not completed whilst on shift.
