Job Overview
We are seeking a dedicated and compassionate Care Coordinator to oversee and organise care services within a care home specialising in senior care. The successful candidate will play a vital role in ensuring the delivery of high-quality care, coordinating with staff, residents, and families to meet individual needs. This role offers an opportunity to utilise your leadership skills and make a meaningful difference in the lives of our residents. The Care Coordinator will be responsible for developing personalised care plans, supervising staff, and maintaining effective communication across all levels of the organisation.

Responsibilities

Organise and oversee daily care operations within the care home, ensuring compliance with organisational standards and regulations.
Develop, implement, and review personalised care plans tailored to each resident’s needs.
Supervise and support care staff, providing guidance and leadership to ensure high standards of care are maintained.
Coordinate with healthcare professionals, families, and external agencies to facilitate comprehensive care delivery.
Monitor residents’ wellbeing, addressing any concerns promptly and effectively.
Maintain accurate records using IT systems for documentation, scheduling, and reporting purposes.
Ensure all activities adhere to health and safety regulations, promoting a safe environment for residents and staff.
Lead team meetings and training sessions to foster continuous professional development among staff members.

Requirements

Proven experience in supervising or managing within a care home setting specialising in senior care.
Strong leadership skills with the ability to motivate and guide a team effectively.
Excellent communication skills to liaise confidently with residents, families, healthcare professionals, and team members.
Valid driving licence is desirable for travel between multiple sites or appointments.
Competent IT skills for managing electronic care plans, documentation, and communication tools.
Knowledge of care planning processes and relevant health & safety regulations.
Previous experience in developing personalised care plans tailored to individual needs is advantageous. Pay: £28,000.00-£32,000.00 per year
