Full job description

Job Summary

We are seeking a dedicated and compassionate Complex Care Support Worker to join our team, providing high-quality, person-centered care for young people with complex needs. In this vital role, you will deliver tailored support to children and their families, ensuring safety, comfort, and independence. Your energetic approach and strong communication skills will help foster positive relationships and promote well-being. This position offers a rewarding opportunity to make a meaningful difference in the lives of children requiring specialized care.

Duties

Assist young people with daily activities such as personal hygiene, feeding, mobility, and medication administration according to individual care plans
Develop and implement personalised care plans in collaboration with families, healthcare professionals, and support teams
Provide emotional support and encouragement to promote confidence and independence in young people.
Use effective communication skills to liaise with families, healthcare providers, and team members about care needs and progress
Support young people safely to appointments, activities, or outings ensuring that community risk assessments have been conducted.
Maintain accurate documentation of care provided, incidents, and updates in IT systems designed for care management
Monitor the young person's health status closely and respond promptly to any changes or emergencies

Requirements

Experience working with children/young people in an acute care setting or within a complex care community service.
Experience as a Healthcare Assistant or Support Worker
Knowledge of child development principles and ability to adapt support accordingly
Experience with conducting clinical observations under the leadership of a Registered Practitioner
Care experience in the field of CAMHS Eating Disorders
Ability to create and follow detailed care plans tailored to each young person's clinical needs
Excellent communication skills to build trusting relationships with young people, families, and colleagues
Proficiency in using IT systems for documentation and reporting purposes
Strong organisational skills with attention to detail in recording care activities and updates
In depth knowledge of Safeguarding, Child Protection and the Duty of Candour
